Credentialing

In order to provide services to Keystone Mercy members as a participating provider, practitioners must:

1. Be credentialed by Keystone Mercy (1-800-642-3510)
2. Sign a Participating Provider Agreement (contract) (1-866-546-7972)

Contracting

If you are not currently contracted with Keystone Mercy and are not joining a group already in our network, please call 866-546-7972 to obtain the appropriate Keystone Mercy Participating Provider Agreement.

Note: The credentialing process and contracting process can occur simultaneously.

Credentialing

Keystone Mercy maintains and adheres to all applicable state and federal standards and regulations, Department of Public Welfare (DPW) requirements, and accreditation requirements governing credentialing and recredentialing functions. Call 1-800-642-3570 for more information.

The following types of practitioners require credentialing and recredentialing:

• Allied Health Providers (Physical Therapists, Occupational Therapists, Speech Therapists)
• Audiologist
• Certified Nurse Midwife
• Certified Registered Nurse Practitioner
• Chiropractor
• Dentist
• Oral Surgeon
• Physicians (DOs and MDs)
• Podiatrist
• Registered Dietician
