Credentialing - Become a Participating provider

In order to provide services to Keystone Mercy members as a “participating provider,” practitioners must:

  1. Be credentialed by Keystone Mercy
  2. Sign a Participating Provider Agreement (Contract)

Credentialing
Keystone Mercy maintains and adheres to all applicable state and federal standards and regulations, Department of Public Welfare (DPW) requirements, and accreditation requirements governing credentialing and recredentialing functions.
The following types of practitioners require credentialing and recredentialing:

Allied Health Providers (Physical Therapists, Occupational Therapists, Speech Therapists)
Audiologist Oral Surgeon
Certified Nurse Midwife Physicians (DOs & MDs)
Certified Registered Nurse Practitioner Podiatrist
Chiropractor Professional Counselor
Dentist Therapeutic Optometrist
Licensed Social Worker/Licensed Clinical Social Worker Registered Dietician

Contracting
If you are not currently contracted with Keystone Mercy and are not joining a group already in our network, please contact Catherine Killian at catherine.killian@kmhp.com or 215-863-5684 to obtain the appropriate Keystone Mercy Provider Agreement.

Note:  The Credentialing process and Contracting process can occur simultaneously.
