Heart Failure (HF)

When you have heart failure, your heart muscle is weak and fluid builds up. Keystone Mercy Health Plan wants to help you live a healthy and active life. With treatment, heart failure can be controlled. Without proper treatment, heart failure can:

  • cause your ankles to swell
  • cause you to gain weight
  • make it difficult for you to breathe
  • make your heart work harder
  • make your energy level lower
  • be very dangerous to your life

The Keys to Your Care program will help you learn about Heart Failure and will help you understand about the medicines you are taking.Keystone Mercy Health Plan has a special program for members who have heart failure. Call our Care Coordination department to sign up for the program. The number is 1-800-573-4100. We want to help you live a healthy life. Call to speak to a Care Manager today.

The Care Coordination program will help you learn about heart failure and will help you understand about the medicines you are taking. The program will also help explain the importance of weighing yourself and avoiding salty foods that can cause you to retain fluids (get puffy feet, "hold water").
