Legal StatementThis Legal Statement discloses the information gathering and dissemination practices of KEYSTONE MERCY HEALTH PLAN. KEYSTONE MERCY HEALTH PLAN can be contacted by mail at 2404 Park Drive, Harrisburg, PA 17110, or by e-mail at webteam@amerihealthmercy.org. 1.
