Keystone Mercy reaches out to members with special health needs to ensure they have access to care and know the importance of regular doctor visits.
Community Outreach Solutions Team (COS)
The COS team is Keystone Mercy's "feet on the street". Many times, our most vulnerable members are also the most difficult to reach. Our dedicated COS team members make home visits and help our members schedule office visits for preventive care, as well as important screenings. This team also arranges transportation and addresses any other barriers to care that the member may have.
Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Team
Keystone Mercy Health Plan works hard to ensure that every child is a healthy child. In 2009, Keystone Mercy launched an outreach team to educate and encourage parents to take advantage of the Early and Periodic Screening, Diagnosis and Treatment (EPSDT) services available to children on the plan. The goal is to ensure children receive important health services that can prevent impairments to their growth and development – before problems occur. This includes age appropriate, periodic well visits for all children, including adolescents and young adults up to age 21. The team uses telephonic outreach to contact parents or guardians to remind them about the services their children need at their current age, schedule doctor appointments, and arrange for transportation.
The Retention Unit was developed to help Keystone Mercy Health Plan members navigate issues surrounding eligibility with the Department of Public Welfare. Through a unique partnership with the Pennsylvania Department of Public Welfare, the Retention Unit routinely assists members with accessing their case worker and maintaining eligibility. In collaboration with Welfare Pride, the Retention Unit has established expedited processes that help insure that case workers and County Assistance Offices provide the necessary services to our most vulnerable members.
